Permission for Course Enrollment

At Pittsburgh Lifeguarding of the South Hills, we follow all the guidelines of the American Red Cross for aquatics, lifeguarding, CPR, AED or first aid classes. Whether you are an adult or below 18 years of age, we need your or your guardian’s approval before enrollment. Please download our waiver and release form and submit it for registration process.

Everyone is required to fill out our Registration Form/Waiver Forms prior to taking class. If you are not 18 years of age, at the time of class a parent or guardian must complete your form.  

Students taking blended learning must bring their certificates that they have completed their online portion.  

Students under the age of 18 must bring their signed paperwork for the class (Parent or Guardian must sign for students under 18).
